Note

• English command is a must
• If questionnaire's display is different from what usuallly generated by the system, highlight it
• It is ok if the consultant use either list or table for grid questions in the document (whatever can be achieved fast from what given by the customer)
• If exact text format (font size, font style...) should be kept, highlight it
• Sub question's index starts from 0, answer option value starts from 1 (if no special values are mentioned)
• Any discussions regarding the questionnaire in DK (if there are) must have a conclusion before sending the document to VN
