If questionnaire's display is different from what usuallly generated by the system, highlight it
It is ok if the consultant use either list or table for grid questions in the document (whatever can be achieved fast from what given by the customer)
If exact text format (font size, font style...) should be kept, highlight it
Sub question's index starts from 0, answer option value starts from 1 (if no special values are mentioned)
Any discussions regarding the questionnaire in DK (if there are) must have a conclusion before sending the document to VN
